General & Family Medicine

The first — and most important — point of contact for new symptoms, routine healthcare and everyday medical questions.

What this appointment is for

A focused consultation on a new or ongoing medical problem. Suitable for most routine concerns where you'd normally see your family doctor.

What's included
  • New symptom assessment
  • Follow-up consultations
  • Medication reviews & renewals
  • Second-opinion style reviews
  • Investigation review
  • Specialist referral assessment
  • Routine physical examinations
  • Minor injury assessment
When to book something different

If you have several or complex problems to raise, ask for an extended appointment where available. Emergencies should go to 911 or your nearest emergency department.

Preventative Medicine

Detecting risk before disease develops is one of the most valuable things primary care can do.

What this appointment is for

A comprehensive, scheduled assessment to understand your current health, identify risks, and develop a plan to reduce them. Usually 30 minutes or longer.

What's included
  • Preventative health assessment
  • Blood pressure assessment
  • Cardiovascular risk assessment
  • Diabetes risk assessment
  • Weight & metabolic health review
  • Age-appropriate screening
  • Lifestyle, nutrition & exercise review
  • Smoking cessation support
  • Alcohol risk assessment
What follows

Where clinically appropriate, the appointment may lead to blood tests, screening investigations or specialist referral. Results and plans are discussed in a follow-up consultation.

03  /  08

Chronic Disease Management

Long-term management of conditions that need consistent, joined-up care over years, not single appointments.

Conditions commonly managed
  • Hypertension (high blood pressure)
  • Hyperlipidaemia (cholesterol)
  • Type 2 diabetes & prediabetes
  • Asthma
  • COPD
  • Cardiovascular disease
  • Thyroid disorders
  • Obesity & weight-related health risks
What a review usually involves

Symptom review, examination where appropriate, medication adjustment, review of recent investigations, and planning the next steps — including whether anything needs escalation.

04  /  08

Mental Health

Dedicated assessment and management of common mental health presentations, with clarity about when specialist support is appropriate.

Areas commonly addressed
  • Anxiety
  • Depression
  • Stress & burnout
  • Sleep disorders
  • Initial assessment of attention concerns
  • Memory concerns
  • Work-related mental health problems
  • Medication monitoring & review
What this appointment is for

A longer appointment (typically 30 minutes) is helpful for mental health reviews — enough time to talk, assess, and plan next steps together.

When specialist services are needed

Complex or emergency psychiatric presentations may require specialist or urgent services. If you are in crisis, contact 911, go to your nearest emergency department, or call Ontario's crisis line.

05  /  08

Women's Health

Preventative and reproductive care across every life stage.

  • Menstrual problems
  • Menopause assessment
  • Hormone replacement therapy (HRT) review
  • Sexual & reproductive health

At the appointment

A chaperone can be arranged for any intimate examination. If you have a specific concern, mention it when booking so the right length of appointment is allocated.

06  /  08

A structured approach to the areas of health men are most likely to defer.

Areas of care
  • Prostate & urinary symptoms
  • Erectile dysfunction
  • Sexual health
  • Cardiovascular risk assessment
  • Metabolic health
  • Mental health
  • Age-appropriate preventative screening

Investigations & referrals.

Where clinically appropriate and permitted within the healthcare system in which care is provided, a consultation may lead to further steps.

These include laboratory investigations, diagnostic imaging, cardiovascular investigations, specialist and hospital referrals, allied healthcare referrals and preventative screening programmes.

Investigations and referrals are based on individual clinical assessment and are not automatically provided following a consultation.
